Open Photoshop and set
Width: 500
Height: 145
Resolution: 100
Make a rectangle and press CTRL + T and move the cursor near the bottom right of your rectangle, so it turns into a curved arrow. This arrow allows you to rotate the object.
While holding Ctrl + Alt, drag the rightmost point upwards, until the shape resembles a diamond.
